If you are considering weight loss surgery, the number that really matters is your body mass index (BMI).

If you are considering weight loss surgery as a way to lose weight, you may be wondering, “How much do I have to weigh to be a candidate for surgery?” But the number that really matters is your body mass index (BMI), a measure of body fat based on height and weight-not your actual weight.

“We’re not going by weight; we rely on BMI ”, says Ayotunde Adeyeri, MD, a certified bariatric, advanced laparoscopic and general surgeon at Raritan Bay Medical Center in Old Bridge, New Jersey. “It’s a more accurate measure of how much work your heart does to support your body.”

The number most bariatric surgeons look for is a BMI over 35 with at least one comorbidity, such as diabetes and high blood pressure. This is when the discussion of surgery can usually begin.

“These are the general criteria,” says Dr. Adeyeri. “But every patient is unique, so the exact indications for surgery may be a little different for everyone.”

For reference, according to the Centers for Disaster Control and Prevention:

  • BMI between 18.5 and 25 is considered normal
  • A BMI between 25 and 30 is considered overweight
  • A BMI over 30 is considered obese
  • A BMI over 40 is considered morbid obesity

By the time most patients come to see it, they have already tried several ways to lose weight over many years, says Dr. Adeyeri, who performs around 100 gastric bypass surgeries each year. They tried and tried but couldn’t maintain the weight for the long term, he says.

“It’s not their fault. It’s a disease, ”says Dr Adeyeri. “Fifty years ago we didn’t classify obesity as a disease, but we do now. Once you have the disease, we recommend that you seek help.

How to Prepare Your Body and Mind for Weight Loss Surgery

Besides reading about the surgery and the results, there are other ways to prepare your body and mind for weight loss surgery. These include:

  • Stop smoking
  • Start tracking your food and water
  • Drink more water and limit all other drinks
  • Start an exercise routine, however small
  • Make a commitment not to gain extra weight
  • Focus on your mental health too
